About the Book Information Form

When you place an order with us, you must fill out our Book Information Form to provide all the necessary data to help us catalog your book accurately. Below, you will see a list of the information asked for in the form, both required and optional fields. Required fields are denoted with (R).

Client Information

  • Contact name (R)

Cataloging Package

  • You will need to make a note of what you purchased (PCIP Data only or MARC records too)
  • If you also purchased MARC records, please make a note of which type of record you bought (Print or Ebook)
  • All of the above information is required

Title & Author Information

  • Book title (R)
  • Book subtitle
  • Series information (series title; sequence number if applicable)
  • Primary author (R)
  • Primary author’s publication history
  • Primary author’s country of residence
  • Additional authors
  • Other contributors (editor, illustrator, etc.)

Identifiers

  • LCCN/PCN
  • Formats in which the book is available, even if there’s no ISBN for some of them (R)
  • ISBN(s)
  • Format to which each ISBN applies, if ISBNs are provided (R)

Publication Information

  • Publication year (R)
  • Publication status (published or not yet published) (R)
  • Projected publication date
  • Publisher name (R)
  • Publisher location (city & state/province) (R)
  • Edition (if applicable)
  • List prices for each format (only if MARC records are purchased)
  • For published books only: Page count & dimensions (height in inches or centimeters)

Content Details

  • Language in which the text is written (if not English)
  • Supporting material (index, bibliography, audio CD, data CD, etc.)
  • If there’s an index/bibliography, does it appear in the ebook version (R)
  • Does the book include inserts (R)
  • Literary form (fiction, nonfiction, mixed forms, nontextual) (R)
  • If mixed form, details about the extra content
  • Genre (fiction only)
  • Is the book aimed at children or young adults (R)
  • Brief book summary (100 character limit; only for children’s & young adult books) (R)
  • Is it a biography (R)
  • If it is, what type (autobiography, individual biography, collective biography, other) (R)
  • Are there any illustrations (R)
  • Geographic locations that play a large part in the book

MARC-Specific Content Details

  • These options apply only if you purchased a MARC record
  • MARC-specific literary form (novels, letters, essays, etc.) (R)

  • BISAC subject headings (you may suggest them, but we retain sole discretion over which headings to use)

  • Book description (not the back cover blurb, but a brief explanation of the book’s primary content) (R)

    • For PCIP data only, this includes general, adult, young adult, juvenile
    • For PCIP + MARC Record Add-On or MARC records only, this includes general, adult, juvenile (ages 0-15 years), primary (ages 6-8 years), pre-adolescent (ages 9-13 years), adolescent (ages 14-17 years)
  • Grade level (for children’s and young adult books only)

Book Summary for WorldCat

If you would like to include a summary that will show up in WorldCat, you may do so. The summary is limited to 250 characters and cannot include any formatting, line breaks, or special characters.

The summary will also be uploaded to SkyRiver, but that catalog has no public interface.

We do not proofread your summary, so please make sure it’s written exactly how you want the public to see it.
